There are a few places to look at pay rates in the airline industry, but pay rates don't always compare when work rules and other contract language applies. Fill out the information below with your average monthly paychecks. If, like at ASA, the 1st of the month check is the base check and the 15th check is the overage and per diem check, combine them into one entry. The values do not have to add up to the Gross Total, the other fields I added for reference.

Aircraft/Seats Aircraft most commonly flown.
Pay Rate $:  Your hourly pay rate.
 
  Monthly Pay Check Stub, use gross amounts not taxable income. Try to fill in as much as possible. The gross amount is the most important.
 
Total Monthly Earnings $: Gross Amount.
 
Regular Hours Amount $:  ...hours regular flown for month 
Premium/Over/Under Amount $:  ...hours 'premium' flown for month 
Taxable Per Diem $:  ...hours Taxable Per Diem    
Untaxable Per Diem $:  ...hours Untaxable Per Diem 
Health Care $:
Union/Association Dues $: Any professional dues per check.
Other $: Other fees aside from taxes that affect gross amount.
